Registration

When your child comes to the carnival or to AWANAź we ask that you fill out a registration card. This card consists of the child's name, age, birthdate, address, telephone number, and parents names. This card is important because it enables us to contact you throughout the year (especially in the case of an emergency) as well as give your child their birthday cupcake. We are also now asking for every parent to fill out a medical release form for each child who attends AWANAź. This information will allow us to seek professional medical help for your child in the case of an emergency. Please be assured that safety for your child is our number one priority. We will attempt to contact you if any medical emergency happens. This form simply protects your child against having to wait for medical attention until you can be contacted. If your child is going to be taken to club by another adult, please fill out this form and send it with your child. We do not give your information to anybody else or use it for any purpose outside of AWANAź. We have a registration booth at the carnival and preregistration at church on Sunday mornings.
